Immunization Record (Ohio)
This report creates an ImpactSIIS Immunization Record for a specific patient. Search for the patient first.
To create this ImpactSIIS Immunization Record, search for the patient and then click the Reports > State Reports > Patient Specific > OH Immunization Record link. The ImpactSIIS Immunization Record form displays for that patient in a new browser tab. To print the form, use the browser's print function.
